What kind of effect do gelatin preparations have?

Arthritis and motive device degenerative diseases are nowadays the general example of rheumatic diseases are  one of the most common clinical frames in doctor’s practice. Knee and hip joints are mainly affected; Tissues like tendons and cartilages are laterally supplied or have a total lack of blood circulation and due to their slow metabolism they cant react to the Noxeni not even to inflammatory processes or regenerative processes.

Can gelatin preparations relief the disease symptoms and support the therapy?

In the recent study patients with joints degenerative problems received three gelatin preparations, which were compared with a proof substance based on the egg white.

Pathogenically the arthritis is differenced in primary and secondary. In the first ones the causes are mostly unknown. This means that any atiological factor could be responsible. (7)

On the other hand a series of causative factors like for example: mechanical overcharge, acute and chronic upsets, metabolic diseases, joints inflammatory processes in repairing stare and also those than conduct to the secondary arthritis unchaining. (14)

This slow and degenerative process takes to considerable discomforts, which makes perceptive by the different upsets and kind of pains during the different stages of the arthritis.

AN AFTER AND BEFORE THERAPY IS A PRIORITY IN THE SYMPTOMS ORIENTATION

For the arthritis therapy there are a series of available measures, which after and before are priority to determine the symptomatic treatment.

In conventional medical therapies, these measures come in the critical phase of the arthritis. Also the non-steroidal, anti-inflammatory and anti rheumatic come intercalate which conducts in a short time to a pain relief and inflammations inhibition. (5)

Eventhough it’s long-term use it accelerates its degenerative process and its application is associated with secondary effects. On the other hand a constant and individual psychotherapy is tried to reach the symptoms relief. The operative process its found and the end of the treatment’s sequence.

 
INCREASING INTEREST IN THE “ CONDRO-ROTECTIVE THERAPY.”

Because of the increasing for a more specific arthritis therapy, some years ago the condroprotectives were intercalated as a basic therapy of joints diseases, which at the same time are phatmaco- stabilizers of the cartilages.

These must inhibit the catabolic formation in the cartilage mass, to improve the nutrients and the energetic supply of the cartilage tissue and finally to decrease the joints damage progress.

Such condroprotective measures have found in last years an open door in therapy, such as the incursion of preparations based on gelatin and partly combined with l-cisteine.

The gelatin’s obtained of collagen substances from animal origin. It’s distinguished by a high content of different amino acids: hydroproxiline, hydroxilisine and arguinine, which have an important roll in the collagen biosynthesis. (6, 8)

It’s highly known and also widely described in literature that the constant gelatin ingestion has a positive effect in hair and nails growth. (9, 11, 12) There also exist empirical and medical studies (1, 6, 10, 13) showing that the containing gelatin products have an important roll in degenerative joints diseases therapy, so they protect and decrease pain.

In the next description there must be proved all knowledge got until now, and consider the efficiency of taking the three different gelatin preparations and a proof substance, based on egg white in patients with arthritic syndrome.

PATIENTS AND APPEARED DISCONFORTS

From the 52 selected patients (24 women and 28 men in an average age of 56) with degenerative hip problems and, or in knees joints, 10 of them showed a partial coxatrosis.
31 showed degenerative problems in both hip joints and 21 of them a marked arthritis in knees joints.

More from the half of these patients have had this disease for five years, but only the 10% showed some discomforts before the first two years. With help of radiological methods the osteoarthritis were diagnosed scarcely until their third grade. Dealing in all cases with an active arthritis form.

The table 1 gives a global appreciation from the most outstanding clinical results applied to the whole patients.

 
THERAPY SCHEME

In a double experiment controlled with parallel groups, the next substances were compared among them.

a) Hydrolized highly pure collagen obtained through enzymatic hydrolysis
The same al gelatin has a high amino acid portion (Arginine, glycine hydroxiproline, proline and hydroxilisine.

b) Gelatin
c) Gelatin + glycine4 + CaHPO 2H 20
d) Egg white

In every form the four substance presented, the ingestion was presented in a random continuous way and for more than 60 days in each case, with a free treatment interval of approximately two months between each other.

Patients ingested daily 10 gr. of substances in form of 20 tablets (each one weight 0.5g). Distributed during the day. The analgesic and anti-inflammatory substances, which were already ingested before the treatment in different portions by patients could be administered during the treatment in controlled dosage. However in the last five years, the anti arthritic ingestion was excluded during and before the treatment.
 

 
INVESTIGATION CRITERIA

As appraisal criteria for a successful treatment the next requirements were taken as pain signals.

These subjective criteria were valuated by patients in:

The initial registers that were given by patients at the treatment beginning were compared with the final ones.
As a complementary general measure the register sums are those that show in a list all pain aspects, this is all the discomfort under a same balance. These must globally present the correspondent point to the life quality evaluation.

Moreover it was also verified at the beginning and final of every study and in all levels the mobility of interested joints, by measuring the angle between the base and the final position. Also it was also determined and selected as a parameter, the average consume of efficient analgesic substances (day tablets) at the ending and beginning of each treatment stage.

It was determined to each patient after and before the treatment closing, the globular sediment, transamino acids value, acid and alkaline phosphate, antibodies in serum against collage kind I, II and III.

 
RESULTS

Pain Conditions

The most important criteria for the treatment success were those where the patients qualitatively valued the pain aspects with a punctuation scale, after and before the of each treatment stage statistically comparing them with a test of Lechmacher.

It can be generally proved that D. (egg white) substance dosage had a significant influence in the diseases frame while the other three treatments showed a significant discomfort reduction (see table 2 & 3)

The compared statistic Chi-Quedrat-test (proof square) differences the register changes among the four administered substances, before the experiment and after the respective stage of the treatment; allowing to see a significant difference with others from D. (egg white) substance in all determined aspects, but weather sensibility.

However, it is notorious that those related to tiredness only the A. substance (hydrolyzed collagen and referring to night pain only B. (gelatin)) are differentiated form D. substance. With respect to weakness criteria, it didn’t show any improvement.

Statistically speaking the four kinds of treatment delimit better one to each other in pain moving criteria. All substances are significantly different among each other except form A, C and D.B. So it can be recognize that in 10 patients it was also an improvement in registers, after the D. (egg white) treatment stage.

In the different therapy course, it was observed a decreasing in the register sum (3 patient statement with respect to A substance, hydrolyzed collagen), in relation with other beginning registers of 17 to 18 outstanding points. Considering that in D. (egg white) substance there was an increasing in the registers of about 2 points.

The register results decrease in an average of 7.4 to 9.0 points with the gelatin different treatments ( A, B and C) in comparison with the initial value. So it can be shown a statistical difference of the three treatments.

It can be observed a clearly significant difference of gelatin preparations in comparison with substance D. (egg white), which influence in general states of pain, can be seen as reliable. (Table 4)

It must be noted that the first twelve patients, which presented a decreasing of their beginning registers by the D. treatment; in more than the 20% this substance was already found before any of the other treatments had begun.

In an apart series in which substance D. was finally administered (here couldn’t be exactly executed) shows that the registers reduction when beginning this treatment were found under the 26% with no exception.

ANALGECIC CONSUME

The analgesic use at the end of every proof cycle is considerably reduced in respect with the exit consume. (See table 49). Upon which the dosage of the D. proof preparations also showed here the same effect (non parametrical to the test of Friedman’s test). In the other treatment it was observed a considerable decreasing in the analgesics and anti-inflammatory ingestion in the nest way: A <B >C (percentage to a mayor decreasing).

As positive marginal observations, it was possible to confirm in some patients during the whole treatment a weigh reduction.

Further on there was a relief in the spinal column pains just as an improvement of the general well being. In the same way with these observations a certain sequence could be determined B <A <C (with less effect)

Brief Information About Gelatin

Gelatin Production:

LGelatin’s a pure protein obtained from substances based on collagen, such as bones tissues and skin.

These industrially obtained products are subjected to a special purification and a particular treatment.

The gelatin and hydrolyzed gelatin are obtained by technical, enzymatic hydrolysis.

Gelatin is a food obtained from a special technology. Gelatin has a basic roll in food industry, because of its smell and its jelly flavor. Due to its consistence it can be used in desserts: gelatin cakes, prepared jellies, candies and diet foods.

In pharmaceutical, gelatin capsules are very requested because of their elegant presentation.

In the other hand gelatin such as hydrolyzed gelatin serve for the tablets and sugarcoated pills manufacture, such as Coaten and micro encapsulated of sensitive substances.

Special chemical modified gelatins are used as plasma expander. For special areas the foamy gelatin tampons are convenient as homeostatic

It’s basic in photographic area. Synthetic materials that have special technological properties are not able to substitute this natural product.

The newest ones are the special gelatins, which can be used in laminal or injection projects manufacturing.

 

MOBILITY

At the end of the study, the knee joint and hip movement extension could be only compared in contrast with the initial state. So then in the hip joint flexion, it was observed a little improvement (an average of 71.7° to 73.5°).

The movement symptoms in other stages stayed with no change, which means that they didn’t get worse, radiological changes were not registered, neither in negative nor in positive sense.

In the same way, the chemical clinical parameters are only modified in an inconsiderable way during the study. Such as blood sedimentation speed, amino acid transference and phosphate values

DISCUSSION

In all recruited patients, it was set at the end of the proof period, a comparable statistic of differential improvement to the proof parameter.

Generally substances based on gelatin can be significantly differentiated from substances containing egg white D.

A theoretical corroboration of the positive results, gives into consideration that through the prevention therapy of the cartilage, the enzymatic and, or the mechanical forming and the irritant factor liberation are reduced.

In this way we can get to the analgesic effects individual observation. It may be conseivable for others that through the gelatin preparations dosage, it’s built in the body an amino acid pool that notably improves the cartilaginous tissues nutrition and due to this condrocytes change position.

SUMMARY

The three-gelatin preparations efficiency and an egg white containing substance were probed in double conditions, in patients that suffer from the first to the third grade of degenerative conditions in the hip and knee joints. The substances were administered according to the random scheme by a period of two months each one and the symptom relieving in the related diseases were evaluated.

In addition to the use of analgesics just as the movement of affected joints were determined as treatment criteria. It was observed in almost every patient a symptom relief after the gelatin preparation administration

It was principally demonstrated in the decreasing of the total registers comparing the previous pain state with those respective periods at the final of the treatment, just as the general analgesic ingestion reduction. The three gelatin preparations could be statistically speaking clearly distinguished from the proof of the substance based on egg white, which didn’t showed practically any effect with respect to the evaluated treatment criteria.

These data further confirm the observations, that gelatin preparations could be benefic in the treatment of joints degenerative process (arthritis) and maybe relief related symptoms.
